What you’ll do:
We have two different case management focuses available! Both are a great fit for someone interested in social service, criminal justice, reentry, or non-profits. Our case management positions will support our mission by working with an experienced culture-intentional team who are committed to helping our clients successfully navigate the justice system and reenter society.

Our REENTRY WORKS CASE MANAGER position is responsible for providing workforce development programming and ancillary service and supports to individuals referred to our reentry programming. The Reentry Works Case Manager will work with referred participants on an individualized employment plan, deliver a series of one-on-one lessons covering employment preparedness education and financial literacy, deliver an evidenced informed cognitive behavioral intervention curriculum, and provide individuals with job search and retention support.

The REENTRY COORDINATOR position is responsible for providing case management services and programming for the Working Toward Change program participants, determining eligibility, conducting initial assessments, aiding clients in developing an individualized employment plan, preparing participants in their return to the community by delivering and connecting them to appropriate services, and providing ongoing case management and employment assistance to clients, post-release. This position also conducts groups and individual sessions for workforce development, essential life skills, and cognitive curricula. The Reentry Coordinator must hold and maintain a clearance with the Berks County Jail System, which will be processed following a conditional offer.
 
Special Requirements:

REENTRY WORKS CASE MANAGER
Must possess and maintain a valid driver’s license, have access to a vehicle and be willing to travel to and work at various locations as needed in order to conduct agency business.  Some locations require working within the confines of a secure facility and in close contact with residents thereof.
 
Must have valid personal vehicle insurance, including providing proof and an acceptable motor vehicle record check.
 
Bilingual: English/Spanish with skill in oral and written communication, including Spanish communication is a plus. Proven proficiency with this skill will result in an increased compensation offer.
 
REENTRY COORDINATOR
Must possess and maintain clearance authorization through the Berks County Jail System.  Connections Work will process the clearance request.
 
Must be able to perform routine physical duties including: ability to lift and carry files and office documents (up to and including 25 lbs.), to traverse stairs regularly, to reach overhead at full arm’s length regularly, to pull facility doors (up to 30 lbs.) throughout the day, to sit/stand for up to three (3) hours at a time without a break, to drive two (2) hours or more in duration.
 
Must possess and maintain a valid driver’s license, have access to a vehicle and be willing to travel to and work within the Berks County Jail and other applicable locations to conduct agency business.

Active Pennsylvania Notary Public Commission is a plus.
